1. Who Can Enter: Limited to legal residents of the 50 United States and Washington DC, age 18 or older as of December 1, 2009. Minors under the age of 18 must obtain the consent of their parent or legal guardian in order to enter the contest. Only one registrant per household is accepted. All federal, state, and local laws and regulations apply. Void where prohibited or restricted.
By entering this Sweepstakes, participants agree to be bound by these Official Rules and the decisions of the Sponsor and/or its agents. Odds of winning prize depend on the number of eligible entries received. BY PARTICIPATING, ENTRANTS AGREE TO ALLOW US TO PERIODICALLY SEND NOTIFICATION OF SPECIAL OFFERS AND PROMOTIONS VIA E-MAIL. IF AT ANY TIME YOU DECIDE THAT YOU DO NOT WISH TO BE CONTACTED FURTHER ABOUT SPECIAL PROMOTIONS VIA E-MAIL, FOLLOW THE INSTRUCTIONS IN THE E-MAIL.
2. How to Enter: Individuals receive one entry into the TopMauiRestaurants Sweepstakes by completing the registration form on TopMauiRestaurants.com. Only one registration per household is accepted.
For those individuals not interested in entering online, they may enter the sweepstakes by sending a postcard with their full name, postal address (including: street, apartment/suite number, city, state and zip code), phone number, and email address to: TopMauiRestaurants Sweepstakes, c/o Maui Media LLC, 1215 S. Kihei Road O-155, Kihei, Maui, HI 96753. Only one registration per household is accepted.
The mail-in entries must be postmarked December 1, 2009 through December 1, 2010, and must be received no later than 5 p.m. Hawaiian Time December 15, 2010 (the “Eligible Mail-In Entries”). Only one entry per household per day is accepted. Any mail-in entry improperly dated will be disqualified. Any mail-in entry that is illegible, incomplete, late or does not carry proper postage will be disqualified.
3. Sweepstakes Dates: The Sweepstakes will begin on December 1, 2009. The prize drawing will be held based on number of sweepstakes entries through the last day of each month in 2010 (log ins & registrations through the last day of each month plus Eligible Mail-In Entries).
Subsequent sweepstakes may be conducted in the future. There is no carry-over of sweepstakes entries from one sweepstakes period to the next. We may conclude the sweepstakes program at any time.
4. Prizes: The total retail value of all sweepstakes prizes is $300.00.
Twelve (12) Prizes: $25 Amazon.com Gift cards/gift certificates – one drawn each month starting in February 2010
5. Prize Drawings: Winners will be selected at random by Maui Media, LLC. based on entries obtained via log-ins & registrations from December 1, 2009 through December 31, 2010 and Eligible Mail-In Entries. The prize drawing for the sweepstakes will be held on or the 15th of the following month. (For example, January 2010’s prize will be drawn on or around February 15, 2010.)
6. Odds of Winning: Each entrant’s odds of winning depend on the entrant’s number of entries via log-ins & registrations from December 1, 2009 and Eligible Mail-In Entries relative to the total number of entries received via log-ins & registrations from December 1, 2009 and Eligible Mail-In Entries.
7. Winner Notification: The winner will be notified via e-mail within 24 hours of the prize drawing (on or about the 15th of each month beginning February 2010 and concluding January 2011). Winners will be required to respond to the e-mail to acknowledge receipt of the prize. If no response is obtained within 7 calendar days of notification, attempts will be made to reach the winner by phone or mail. If no response is obtained from the winner by January 15, 2011,the prize will be awarded to a replacement winner drawn at random. Maui Media, LLC’s decisions about winners are final. Prize is not transferable.
8. Limits of Liability: Maui Media, LLC has no responsibility whatsoever for injuries, losses, or damages of any kind that result from acceptance, possession, or use of any prize. Maui Media, LLC assumes no liability for lost or misdirected registration forms or survey submissions whether due to technical or computer malfunctions or otherwise. Maui Media, LLC assumes no liability for technical or computer malfunctions of any kind. Maui Media, LLC reserves the right to cancel the sweepstakes at any time if our website is infected by a virus, the website is hacked or the integrity of the sweepstakes is compromised in any way.
9. Change to Rules: These Official Contest Rules are subject to change each month that the sweepstakes is conducted. Participants should review the Official Contest Rules each month.
10. Winner Responsibilities: Winners are solely responsible for the reporting and payment of any tax liabilities incurred. Winners must submit an affidavit of eligibility and compliance with the Official Contest Rules.
11. Winner Publicity: Maui Media, LLC may wish to publish the first name, first letter of last name, and location (city/state) of winners, and any quotation that the winner is willing to provide on our websites. However, Maui Media, LLC stands by our commitments of confidentiality related to personal identity and will request written permission from each winner prior to publicizing any names on the website.
